Appointment Cancellation Policy: Failure to keep your scheduled appointment hinders our ability to provide the best care for you. In order to restrict missed appointments, we have implemented an Appointment Cancellation Policy. We ask that in the event you need to cancel your appointment, you call at least 24 hours prior to an appointment. This will allow us the opportunity to offer the appointment to another patient. Late cancellations due to illness or family emergencies are excluded from this policy.

Repeated late cancellations and missed appointments are disruptive to the optimal delivery of care to you and to our other patients. As a result, 3 late cancellations or missed appointments over the period of 12 months may result in the discontinuation of your care with Legacy Eye Care.

Fees: At Legacy Eye Care, failure to give 24 hours notice prior to cancellation will result in a “Missed Appointment/No Show Fee”. This fee cannot be billed to your insurance company and will be your direct responsibility. The fee is $50.00 per incident.

Late Arrival Policy: A grace period of 15 minutes will be allowed for scheduled appointments. If you arrive after the 15 minute interval, you will need to reschedule the appointment.
